Patient Electronic Notes Update

RevolutionEHR now has fields that contain “searchable” text fields. A few of the fields are counted for MU, and many others are not. The requirement at this time is not for these fields to be searchable over multiple encounters or multiple instances of entries.

The following searchable notes SHOULD count for MU since they are within an encounter and their content is locked from editing once an encounter is signed and completed.

  • Encounter RFV
    Select a Patient, go to encounters, select encounter (e.g. comprehensive encounter), go to RFV screen.  The Patient and Provider reason for visit text areas are searchable.
  • Quick Add CarePlan add/edit within an encounter
    Select a Patient, go to encounters, select encounter (e.g. comprehensive encounter), go to Assessment & Plan/Plan. Add or edit a “Quick Add” care plan item.  The Description text area is searchable.

The following searchable notes SHOULD NOT count for MU, since they aren’t directly related to an encounter or not uneditable and within a signed and completed encounter.  (When an encounter is signed and completed, the header note content is not locked from being edited.)

  • Encounter Header Notes
    Select patient, go to encounters, select encounter, click on Notes button in encounter header – view notes.
  • Diagnosis Notes
    Select patient and view encounter (e.g. comprehensive encounter), go to Assessment & Plan/Assessment, open diagnosis from Master Problem List, go to Notes slider pane.
  • Reports – Payment Search Result Item Notes
    Reports/Accounting/Payment Search – search for payments, select, open payment, go to Notes slider pane.
  • Appointment History Notes
    Go to More/Reports/Schedule/Appointments, search for appointments, select appointment to show details, click open, go to Appointment Notes slider pane.
  • Accounting Invoice Notes
    Go to Accounting, search for an invoice, select invoice, go to Notes slider pane.
  • Patient Pharmacy – Pharmacy Notes
    Select patient then go to Show More/Pharmacy, select a patient pharmacy, go to Pharmacy Notes slider pane.
  • Contact Lens Rx, Eyeglass Rx, Med Rx, General Med Rx Notes
    Select patient and go to Show More/Rx.
    Select a contact lens Rx, go to Notes slider pane.
    Select an eyeglasses Rx, go to Notes slider pane.
    Select a medication Rx, go to Notes slider pane.
    Create a general med Rx from the medication screen, go to Notes slider pane.
  • Task Notes
    Go to Tasks and create or view a task, view Notes in lower half of screen.
  • Patient Dashboard Notes
    Select patient and view notes.
  • Eyeglass/Contact Lens/Frame Order Details Notes
    Perform an order search and select an order of the 3 different types, go to Notes slider pane.
  • Provider Notes
    Select a Patient, then go to Show More/Providers, select or create a new provider, go to Provider Notes slider pane.
