Patient-Generated Health Data

Overview

EHR technology must enable a user to 1) identify, record, and access information directly and electronically shared by a patient (or authorized representative), and 2) reference and link to patient health information documents.

PI Objective and Calculation

The provider must incorporate patient-submitted data or data from non-clinical settings into RevolutionEHR.

Measure:

  • For more than 5% of unique patients seen by the EP during the EHR reporting period, patient-generated health data or data from a non-clinical setting is incorporated into the EHR.

Denominator for calculation:  Number of unique patients seen by the provider in the reporting period

Numerator for calculation:  Number of patients in the denominator who had patient-generated health data or data from a non-clinical setting is incorporated into their record

EXCLUSIONS: 

  • Any EP who has no office visits during the EHR reporting period

 

Promoting Interoperability Discussion

This objective formally recognizes the value of both patient-tracked information and data that might come from a setting beyond that of another physician’s office.  Examples of sources of data could include wearables (ie. activity trackers), home blood pressure readings, mobile applications, etc.   Examples of non-clinical data could include social services data, advance directives information, etc.

To provide this capability in RevolutionEHR, a new component named External Data has been created.  Accessible through either “Show More” or the menu running down the left side of the screen on the patient dashboard, External Data allows the user to accept or a link to electronic information, name the information, and upload an associated file:

Conclusion

RevolutionEHR provides a straightforward workflow for the Patient-Generated Health Data objective, but providers might find challenge in surpassing the 5% threshold as there simply might not be enough patients aiming to submit this type of information.
